Registration Form

1. Registration will be on a “FIRST COME – FIRST SERVED” basis or until fully booked.

2. In the event of a “NO SHOW”, participants MUST pay the full registration fee; a P1,000.00

penalty for re-scheduling.

3. To complete and confirm your registration, please email your registration form together with the

validated deposit slip to: trainingdiv.ccainc@gmail.com

4. Please make your payments payable to CEBU CONTRACTORS ASSOCIATION, INC. & deposit them

to Metrobank SA# 602-7-60290919-7

5. Once registered we will provide them with the ZOOM meeting details.
